Quiz Add-On

The Gravity Forms Quiz Add-On allows you to quickly create Quizzes that are automatically graded or scored when the form is submitted. The Quiz Add-On introduces a new Quiz Field which allows you to configure quiz questions and answer choices that are presented to the user either as a Radio Button, Drop Down or Checkbox Field. You can configure a Quiz to be graded either on a simple Pass/Fail scale or using a more complex letter grade system that you can customize.

Quizzes allow you to interact with your users and present fun interactive quizzes that include instant quiz scores and answer explanations. Don't want to show the user the results immediately? You have complete control over what is presented to the user when the quiz is completed.

The Quiz Add-On allows you to add as many Quiz Fields as you would like to ANY form you choose!

Install The Quiz Add-On

The easiest way to install the Quiz Add-On is from within your WordPress site itself. If you have properly configured your Gravity Forms license key on the Settings page of Gravity Forms you should have access to the built in Add-On manager.

  • Login to your WordPress admin dashboard.
  • Go to Add-Ons under the Forms navigation.
  • Install and Activate the Quiz Add-On.
  • Get Started!

If you would like to install the Quiz Add-On manually, or have trouble accessing the Add-On manager, you can follow the instructions below:

  • Download the add-on plugin from the Add-Ons download page.
  • Login to your WordPress admin dashboard.
  • Go to Add New under the Plugins navigation.
  • Choose Upload from the Install Plugins sub-navigation.
  • Upload, Install and Activate the Quiz Add-On.

Setup Your Quiz Fields

After installing and activating the Quiz Add-On, you can now add Quiz Fields to your form. This is done by adding a Quiz Field from the Advanced Fields toolbox in the form editor to any form you would like to add a Quiz to.

  • Go to New Form under the Forms navigation or choose an existing form from Edit Forms in your WordPress admin dashboard.
  • Click on the Quiz Field under the Advanced Field toolbox to add a Quiz field to your form.
  • Edit the Quiz Field to configure the available Quiz options.
  • Configure your Quiz Field as a Radio Button, Drop Down or Checkbox using the Quiz Type option.
  • Enter the quiz question in the Quiz Question input of your Quiz Field.
  • Enter the answer choices in the Quiz Answers interface to configure available answers.
  • Select the correct Quiz Answer by clicking on the checkbox icon that appears to the right of each Quiz Answer.
  • If you'd like to randomize the order of the answers that are displayed, you can enable the "Randomize order of choices" option.
  • You may include an explanation of the answer by enabling this option and configuring the explanation.
  • Save your form.

Setup Your Quiz Grading

After adding Quiz Fields to your form you will now need to configure what type of grading you would like to implement for your quiz. When you add a Quiz Field to your form, a new section will automatically be added to the Form Settings that introduced Quiz Settings related to grading.

  • Select Form Settings from the Gravity Forms toolbar.
  • Select None, Pass/Fail or Letter from the Grading option. Pass/Fail allows you to enter a percentage that must be reached in order for the user to "Pass" the quiz. The Letter Grading option allows you to configure more advanced grading that you can customize.
  • If configuring the Quiz Grading to use Pass/Fail you would then enter the Pass Percentage that must be achieved in order for the user to pass the Quiz. You can then customize the confirmation messages that will be displayed if the user Passes or Fails the quiz. These confirmation messages support Gravity Forms merge tags. The Pass/Fail Confirmation message will automatically be displayed AFTER the standard Form Confirmation message when the form is submitted.
  • If configuring the Quiz Grading to use Letter Grading you would then configure the Letter Grades to suit your needs. You can customize the Label and the Percentage that must be achieved in order to receive that grade. You can then customize the quiz confirmation message that is displayed. The quiz confirmation message will automatically be displayed AFTER the standard Form Confirmation message when the form is submitted.
  • Save your form.

That's it! Creating a Quiz using Gravity Forms is that easy.

Display Your Quiz

The Gravity Forms Quiz Add-On uses the standard form shortcode, function and widget in order to display the form. You would insert your quiz into a page or post using the standard methods you already use when displaying a Gravity Form on your site.

The Quiz Confirmation messages that you configure in the Quiz Settings will automatically be displayed after the Form Confirmation message when the form is submitted.

Quiz Merge Tags

The Quiz Add-On introduces several new Gravity Forms Merge Tags that can be used anywhere within Gravity Forms where merge tags are supported. Merge Tags are primarily used in the Form Confirmation message and Notification messages to output the results of a Quiz within the content of the message.

{all_quiz_results} Displays the results of all Quiz Fields that exist in the form.

{quiz_score} Displays the number of correct Quiz answers the user submitted.

{quiz_percent} Displays the percentage of correct Quiz answers the user submitted.

{quiz_grade} Displays the Grade the user achieved based on Letter Grading configured in the Quiz Settings.

{quiz_passfail} Displays either Passed or Failed based on the Pass/Fail settings configured in the Quiz Settings.

Quiz Results

You can view aggregate results of your quiz and filter this data based on date and field selection by going to the Quiz Results. You access Quiz Results by clicking on the Results navigation item in the Gravity Forms toolbar. If you have multiple Add-Ons installed that support Results you will need to rollover the "Results" navigation item and then select "Quiz Results".
