Form Not Displaying

    I have recently moved my site to a new server and fin d that one of my forms is not showing on a page.

    - I disabled all plugins » form not showing,
    - Switched to Twenty Ten theme » Form not showing,
    - Duplicated the plugin » form not showing,
    - Changed the form on the page to another form » that form did not show,
    - Created a new page and embedded the form » form not showing.

    Not sure what is wrong. It was working on my development server but not on the live server. The other two forms in the site do show.

  2. The site is showing a JS error Screenshot which is most likely the source of your issue since this form is using conditional logic.

    Does the form show from the form builder preview area?

    Do you see that error on the development server? Are there any differences at all between the development server site and the new server site?

  3. The form doesn't show in the preview area, and I notice now that the same problem is on the development server (http://174.120.168.220/~auspectc/resources/book-now/). It was showing a few days ago when I had completed it. I'm not sure where the JS error emanates from, and the problem still persists when disabling all plugins, and switching the the twenty ten theme. I haven't added any scripts to any of the pages or to my theme. I am running the most recent version of WordPress. All plugins are up to date. I am happy to provide you with an admin login to have a look if that helps. Many thanks.
