ccphysio - issues with Gravity forms

    Hi,
    My site is at ccphysio.co.nz
    I have set up Gravity forms. The emails generated from the forms are intermittent in reaching the recipient, even though every one of them can be see as received in Forms>entries. I am trying to find out where the breakdown happens.
    Can someone look at the settings and let me know if there is something wrong in the settings? This is not my fiurst set-up of Gravity forms - probably the 50th. All others are working properly.
