checkbox + text entry field

    I have a list of checkboxes. Next to one of the checkboxes I would like to place a text entry field. (For example, the checkbox label would be "magazine" and the text entry field would be for the user to enter the specific name of the magazine.)

    What is the best way to accomplish this?

    I can't imagine that I'm the first person to want such a configuration, but I could not find any relevant info in the docs or in the forums. My apologies if I missed it.

    Only the radio buttons have an "Other" field. Check boxes currently do not. Other people put a conditional field where one of the check boxes is "Other" and then a field appears to write in the other option.
