Create Forms

    When I try to create a form I enter in a Form Discription and hit create form botton and what I get is: Please enter a form title.

    What is going on?

    You probably can't see the other field asking for the form title because of a JavaScript or CSS conflict. Can you please go to Forms -> Settings and Enable No Conflict mode?
