Form entries mixed up

    Hi there, we are using Gravity Forms as a way to let users upload a profile, which can include up to 10 images.

    We just recently noticed that random profile uploads are getting mixed up. For instance, images uploaded by one person are being assigned to another person. There's no obvious pattern to this, except that it seems to occur if two or more users are uploading profiles at the same time.

    But we can't reproduce the issue, which is why I can't say for certain that this is the root of the problem.

    In terms of conflicting plugins and themes - the site has been live for a few months now and we cannot disable plugins and set the theme to the default theme in order to identify any potential conflicts.

    We use a couple of image size reducing plugins - are there any known plugin conflicts that I can check?

    Can you offer any advice here please?

    Any update on this please? It's more or less been 48 hours.

    We would like to get this resolved asap.

    Some further information:

    - the form has 3 steps. Step 2 lets users upload at least one photo and up to 10 photos in total. The image paths are getting mixed up for certain users here.
    - sample of some of the user agents for the profiles being mixed up:

    Without running through the plugin/theme conflict troubleshooting steps and because it is only happening randomly this is going to be difficult to resolve. Out of interest are you using any kind of caching such as W3 Total Cache or WP Super Cache.
