  7. I just had a chance to look at the form and I see what you mean. Does the form work correctly in the form preview? And will it work correctly if you try the Twenty Eleven theme from WordPress? It looks like there is some sort of plugin or theme conflict causing the form not to work properly.

    Hi Chris,
    the form preview works fine and with Twenty Eleven to. I think is a theme conflict, but I don't have any idea how found the bug.

  9. You should contact your theme provider for their support. I had a couple thoughts though. The HTML in the top menu is invalid. Can you hide that menu, or fix the HTML, and see if that has an effect?

    Also, looks like you are using uniform.js. That has a direct effect on all your form elements. Can you disable that and check again? For some reason, the next button and all the drop downs on the bottom have "display:none" added to them, in addition to the class for the 'uniform' script.

    Unfortunately the theme provider has a very bad support! I opened a new ticket and we will see...
    Can you please give me more details how to fix the HTML in the top? Witch part of the code I have to fix?

    For the second problem I have to edit the functions.php and delete this line?

    wp_register_script( 'jquery-uniform', get_template_directory_uri() . '/js/vendor/jquery.uniform.min.js', array( 'jquery' ) );

    Finally I found a solution and now it works!
    I have just two small visualization problems for:

    Advanced Field – Right Container > Last Name and Confirm Email

    The right border is cat!

    Radio Buttons Field

    On the left side the radio button circles are cat!

    How can I resolve this?
