how can i put a text disclaimer with checkbox in form multipage ?

    Hello,
    How can i put a text, with under a check box ? i can't find how to put text only and after a check box. please see image

  2. This might be the easiest choice for you.. a third-party add-on for Gravity Forms that creates a specific field for terms of service.

    It put a field filled in by the customer, and I do not want it?

  4. Okay, then use a HTML block for the terms/disclaimer content and then you can use a single checkbox (set to required) for them to agree to the terms. You can use the form conditional logic to hide the submit button unless the required checkbox has been checked.
