Survey Style Form with Results?

    Hi,

    We'd like to build a form that allows the user to answer a few yes/no questions (basically a survey) and then it would email the results back to the user, with the number of questions they answered "yes", along with some other things. Also, we'd like an email sent to a different email address, dependent on the selection of a user's name inside a drop down menu within the form.

    Your software seems to handle a lot, but can it do these things? Many thanks in advance.

  2. We do have conditional notification routing based on radio/check box/drop down fields. As far as the results in the email, we have merge tags for use from the form fields on the form. The default merge tag is the "All Fields" merge tag, which will show the data entered from the form.

    So if I wanted to say "You answered 8/10 Questions 'Yes'...", I could do that? Or is that above and beyond?

    You would then have to do some custom coding to create such a message. It would instead send a notification currently that just showed each question and the answer they submitted. If you want to count answers, you would want to do something similar to what I've described here:
